Many of the forms you will need to become a provider or make changes are available here to download and complete. Once
completed, email assistance@childcarehlep.com or drop off Mon. – Fri. during the hours of 8:00 AM – 4:30 PM. We also have
an after-hours drop-off box that is located at the office for your convenience.

We meet every 1st and 3rd Tuesday of the month from 1:00 – 2:00 PM. Offered both virtually and in-person at our Joliet office and Kankakee.
